Thank you for considering our office for your oral surgery care. If you’re a new patient to our office, follow along for some information regarding scheduling and office policies. We are excited to meet you and hope to exceed your expectations!
Scheduling Your Appointment
If you are ready to schedule your appointment, we can help you out either over the phone, text, or email. Most of the information we need will be found on the referral form provided by your dentist/orthodontist. We are a specialty office and require a referral from another dental provider for most procedures (except wisdom teeth and facial cosmetics).
Let us know if you would like to schedule for just an exam or potentially an exam with same-day treatment on your initial visit.
Considerations for Exam and Treatment on the Same Day
If you would like same-day treatment, we will also need a recent x-ray from your dentist as well brief review of your health history. Not all patients are good candidates for same-day treatment, so we need to make sure that your procedure is not high risk and there are no medical contraindications for treatment on your initial visit. Below is a non-comprehensive list of conditions where we cannot do same-day treatment. Your overall health and safety are our number one priority.
- you prefer to have an initial consultation
- you would like to have oral sedation
- extraction with high risk of nerve injury
- extraction with high risk of sinus injury
- high risk of other dental complications
- take blood thinners or have a bleeding disorder
- see a pain management doctor
- history of drug abuse
- poorly controlled asthma
- uncontrolled hypertension
- current or history of organ failure
- immunocompromised condition
- history of head and neck radiation
- current or past bisphosphonate use
- current or recent chemotherapy
Finally if same-day treatment is offered to you, you will also need to decide if you would like to be awake or have IV sedation for your procedure. For IV sedation you will be required to fast before your appointment and have a an adult driver with you.
Financial Information
Our office accepts PPO dental insurances as an out-of-network provider. If you provide us with you insurance information, we can generally verify your benefits as well as give you an estimate of what insurance is expected to reimburse. We will help you with the filing your insurance as we want you to get the most out of your benefits.
Payment is due at the time of service and can be made with cash, credit cards, or debit cards. Third-party financing is available through Care Credit and Lending Club, including an option for 6 months interest free financing.

You may fill out our forms ahead of time. They are available as web form or downloadable PDFs. If you haven’t had a chance to fill them out ahead of time, we will have them ready for you at your visit.
Patients planned for treatment will want to review all of our Pre-Operative Instructions. Reviewing the Post-Operative Instructions will help you see what to expect after your treatment.
All patients under 18 year old will need to have their parent or legal guardian.
